Epic Healthcare Management is seeking passionate Registered Nurses (RNs) to fill Full-Time, Part-Time, and Per-Diem positions at Epic Healthcare Management for Rehabilitation and Nursing. As a Registered Nurse Case Manager, you'll play a vital role in coordinating care and ensuring the well-being of our residents.

What you'll do as Nurse Case Manager:

• Provide residents and their families with guidance and support as they navigate healthcare choices and resources.
• Monitor residents' health, administer medications and treatments as prescribed, and ensure their overall well-being.
• Accurately and thoroughly document resident care services in resident and department records, guaranteeing all relevant information is accessible to the care team.
• Uphold strict infection control protocols to protect the health and safety of both residents and staff.
• Collaborate with multidisciplinary teams to address resident concerns and needs, developing personalized care plans that cater to each resident's unique requirements.
• Ensure the delivery of high-quality care by adhering to facility philosophies and standards. Promote resident-centered approaches in all aspects of care.
• Maintain resident confidentiality and trust by handling sensitive information with utmost discretion and professionalism.

Qualifications for Nurse Case Manager:

• Current State Registered Nurse (RN) License.
• Experience working in long-term care settings is preferred.
• Solid computer skills with a working knowledge of MS Office.
• Excellent communication, interpersonal, and collaboration skills.
• A friendly and team-oriented approach.
